Warnings and precautions
Talk to your doctor or pharmacist before taking HerzASS-ratiopharm:

if anticoagulants (for example, coumarin group drugs or heparin, except when the latter drug is used in low doses) are used at the same time;
if you have mild or moderate liver, kidney or heart damage;
if you have had gastrointestinal disorders;
if you have had gastrointestinal ulcers or bleeding from the gastrointestinal tract in the past;
with bronchial asthma or obstructive pulmonary disease, as salicylic acid can cause asthma attacks;
when there are allergic patients who have had skin allergic reactions due to anti-inflammatory painkillers, rhinitis;
the first six months of pregnancy.
If prolonged vomiting, loss of consciousness or abnormal behavior occurs during treatment, the use of acetylsalicylic acid should be discontinued.

Low doses of acetylsalicylic acid inhibit the excretion of uric acid and may therefore provoke a gout attack in individuals who are prone to gout.

In children and adolescents,
Reye's syndrome, a very rare but life-threatening disorder that occurs in children with some viral illnesses (especially chickenpox and the flu virus). Acetylsalicylic acid may increase this risk. Therefore, acetylsalicylic acid is not prescribed to children.

Patients with bronchial asthma, hay fever, swelling of the nasal mucosa (nasal polyposis) or chronic respiratory tract infection (especially if symptoms of hay fever are also present), as well as patients with increased sensitivity to all groups of drugs for pain, inflammation and rheumatism, using HerzASS-ratiopharm 100 mg tablets may cause attacks of bronchial asthma (so-called intolerance to analgesics or asthma of analgesics). If you have any of the listed conditions, be sure to talk to your doctor before taking this medicine. This can happen if your body is more sensitive to other medicines (for example, a skin reaction, itching or hives).

The anticoagulation effect of low-dose acetylsalicylic acid persists for several days after administration, so patients should be warned about the risk of bleeding during surgery, including minor surgery (eg, tooth extraction).

Without consulting a doctor, preparations containing acetylsalicylic acid should not be used for a long time or in large doses.

Other medicines and HerzASS-ratiopharm
If you are taking or have recently taken any other medicines, or you are not sure, please tell your doctor or pharmacist.

Treatment may be affected by the use of HerzASS-ratiopharm together with other medicines:

acetylsalicylic acid can enhance the effect of heparin and increases the risk of bleeding when used together with oral anticoagulants, drugs that reduce platelet aggregation and fibrinolytics;
the risk of bleeding from the gastrointestinal tract increases if corticosteroids are used together or alcohol is consumed;
simultaneous use of drugs for the treatment of diabetes mellitus (sulfanylurea derivatives);
medicines for cancer or rheumatoid arthritis (eg methotrexate);
the concentration of these drugs in the blood plasma increases with simultaneous use of digoxin, a drug used to treat heart diseases, drugs affecting the nervous system, barbiturates and lithium;
medicines for pain and inflammation (e.g. steroid anti-inflammatory drugs);
for the treatment of infectious diseases with sulfonamide and combination drugs containing sulfonamides (eg, sulfamethoxazole and trimethoprim);
triiodothyronine, a medicine used to treat thyroid diseases;
an anticonvulsant with valproic acid;
from high blood pressure (eg diuretics and angiotensin-converting enzyme inhibitors);
medicines against gout (e.g. probenecid);
against organ rejection after transplantation (eg cyclosporine, tacrolimus);
Acetylsalicylic acid should not be used together with other non-steroidal anti-inflammatory drugs. Taking two or more NSAIDs increases the risk of serious gastrointestinal bleeding;
acetylsalicylic acid can increase the concentration of acetazolamide, which can lead to severe acidosis and increased central nervous system toxicity;
in high doses, salicylates can reduce the need for insulin.
Talk to your doctor before taking acetylsalicylic acid. If you are taking acetylsalicylic acid regularly, ask your doctor for advice before taking any other medicines, including over-the-counter medicines.

Pregnancy and breast-feeding Pregnancy
If
you are pregnant, breast-feeding, think you may be pregnant, or are planning to become pregnant, talk to your doctor or pharmacist before taking this medicine.

Medicines containing acetylsalicylic acid should not be used during the first and second trimesters of pregnancy, unless absolutely necessary. If such drugs are used by a woman who wants to become pregnant or by a woman in the first and second trimesters of pregnancy, the lowest possible dose should be administered for the shortest possible time.

All prostaglandin synthesis inhibitors used in the third trimester of pregnancy can cause the fetus:

toxic cardiopulmonary effect (when the fetal ductus arteriosus closes prematurely and pulmonary hypertension occurs),
renal dysfunction that may progress to renal failure with oligohydramnios,
Taking daily doses of acetylsalicylic acid of more than 300 mg immediately before birth can cause bleeding in the brain, especially in premature babies.
All prostaglandin synthesis inhibitors used in the third trimester of pregnancy can cause the mother and the baby at the end of pregnancy:

a possible prolongation of bleeding time - an inhibitory effect on blood coagulation, which is observed when using the preparation even in very small doses,
suppression of uterine contractions, resulting in delayed and longer labor.
Therefore, acetylsalicylic acid should not be used during the third trimester of pregnancy.

Breastfeeding
A small amount of the active ingredient acetylsalicylic acid and its breakdown products pass into breast milk. Since there are no data on the negative effects of the drug on the baby, if the daily dose does not exceed 150 mg (corresponding to one and a half tablets of HerzASS-ratiopharm 100 mg), there is no need to stop breastfeeding. If acetylsalicylic acid is used more than 150 mg per day (corresponding to more than one and a half HerzASS-ratiopharm 100 mg tablets), breastfeeding should be stopped.
